Kate Johnson Osteo is committed to protecting the privacy of patient information and to handling your personal information in a responsible manner in accordance with the Privacy Act 1988 (Cth), the Privacy Amendment Act 2012 and relevant State legislation. This Privacy Policy explains how we collect, use and disclose your personal information, how you may access that information and how you may seek the correction of any information. It also explains how you may make a complaint about a breach of privacy legislation.

From time to time we may make changes to our policy, processes and systems in relation to how we handle your personal information. We will update this Privacy Policy to reflect any changes. Those changes will be available on our website and in the practice.

Collection: 

We collect information that is necessary and relevant to provide you with medical care and treatment, and manage our practice. This information may include your name, address, date of birth, gender, health information, family history and contact details. This information may be stored on our computer medical records systems. We may also collect your medicare and or health fund details for claiming purposes. Wherever practicable we will only collect information from you personally. However, we may also need to collect information from other sources such as treating specialists, radiologists, pathologists, hospitals, other health care providers. We may collect information in various ways, such as over the phone, or in writing, in person in our Kate Johnson Osteo Clinic or over the internet if you transact with us online. This information may be collected by medical and non-medical staff. In emergency situations we may also need to collect information from your relatives or friends.

We are required by law to retain your medical records for certain periods of time depending on your age at the time we provide services.

When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

Kate Johnson Osteo uses social networking and Newsletter services including; Facebook, Instagram and Mailchimp to communicate with the public and our clients. We may collect your personal information when you communicate with us by using these social networking services. These social networking services will also handle your personal information for their own activities. These social networking sites have their own privacy policies. You may unsubscribe from our mailing/marketing lists by opting out at any time or by contacting us in writing.

We also use Google Analytics and Facebook Pixels that use cookies. We use cookies to help our website run effectively. You can choose to switch off cookies on your personal device whilst browsing.

Disclosure of Personal Information

We will treat your personal information as strictly private and confidential. We will only use or disclose it for purposes directly related to your care and treatment, or in ways that you would reasonably expect that we may use it for your ongoing care and treatment and will gain your consent prior to disclosure in these instances. For example requests for x-rays and or communications with your general practitioner and or specialists. 

There are circumstances where we may be permitted or required by law to disclose your personal information to third parties.

We may also disclose de-identified information of our users to third parties for analysis, research and quality assurance purposes.

Some third party service providers used by Kate Johnson Osteo may store your personal information on servers located overseas, however, they must also meet our requirements for privacy and data security.

Kate Johnson Osteo will take reasonable steps to protect your personal information from misuse, interference and loss, and from unauthorised access, modification or disclosure.  Your personal information is stored in a manner that reasonably protects it from misuse, loss and from unauthorised access, medication or disclosure.

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.

Kate Johnson Osteo will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.

In order to protect your Personal Information we may require identification from you before releasing the requested information.
